I don't know when most of y'all get out of school, but in Waxahachie we have one staff work day-tomorrow- and then we're done! I can't believe how fast time has flown! And- now REALLY comes time to swim in paperwork! Some of you may have already turned yours in and are a step ahead of the game. But for those that haven't, I THINK this is what we need to turn in to ASHA:
- Report & Rating Form
- Application for Certification and Membership, $336 fee, and make sure you include the "Application Form" that Dr. Bender signed last May
- KASA form
- Official transcript
- Praxis scores (for those like me who forgot to have them sent when we took it!)
- Journal Selection Form
For TEXAS gals I think we need:
- Report of Completed Internship Form
- Application Form, $163
- Transcript
- Praxis Scores
IF ANY OF YOU KNOW OF SOMETHING ELSE I'M FORGETTING PLEASE TELL ME ASAP! I'm planning on mailing my stuff in the next few days and really hope I've got it all together. Kinda scary, but REALLY exciting! Hope everything is going great for each of you; miss you all!

I recently called ASHA, and they no longer had my application form on hand. So, I filled out a new application form to send along with all my information. I believe Kellie also had to do the same thing. If you want, call ASHA or include a application form just to be sure! Other than that, I think that sounds like a comprehensive list!
Also, for the TEXAS license, it recommends sending in a written statement from your supervisor stating that he/she agrees to continue to supervise you while your waiting for the license. For school employees, it may not be necessary. But, I'm going to send one in just in case! I'd rather have too much information than not enough.
Yes... I have finished everything and mailed it all in and your list looks great... although I don't know what the Journal Selection form is or maybe I just don't remember... but good job on the list! Can't believe our CFY is over!!!
The journal selection form is just the extra form where you tell them what additional free journal you want as a member of ASHA.
I just finished yesterday and will be mailing all my paperwork in as soon as possible! We have a summer institute for continuing education starting Monday and I am leading a session with the other new SLPs in our district. It is all about what speech pathology is and what we do in the school setting. Then I will be attending classes the rest of the week. Oh, and I'm moving out of my parents house this summer!
